b) Athlete Code of Conduct Form.
c) Parent Code of Conduct Form.
d) Photocopy of the athlete’s birth certificate – this is a requirement of the OVA.
2) Uniform sizing.
If you did not attend Madawaska then you MUST try on uniforms and select your sizes and jersey number.
3) Make payment for the club season.
a) Madawaska – if there are any outstanding balances from our teambuilding camps these need to be paid.
b) Registration fees. Our registration fees this vary by age group. Also, our jerseys are identical to last year so returning athletes may be able to reuse their existing jerseys. Our fees are outlined in this Fee Schedule:
This year our fees are to be be paid in no more than two installments, dated October 1 2013 and January 6 2014.
We realize that some families may need to make alternate arrangements. We also have an Athlete Assistance Program this year that supports families who qualify for external funding programs. If you wish to discuss either of these options we ask that you submit cheques as per usual but immediately contact the Treasurer (treasurer@durhamattack.org) to schedule a discussion.
c). Physical Training,. Provide a cheque for $316 payable to ‘Team 12 Training’, dated October 1, 2013. If parents have questions then our trainer Mike Slean will be available at registration to discuss our physical training philosophy and approach, and what this fee covers.
Questions: If you have questions please contact your coach, or the executive.
Hello, my daughter is interested in playing volleyball, does your club offer recreational levels? Is it too late to tryout for a team?
ReplyWhat would the fees be for a 12 year old who will be turning 13 in December?
Thank you
