Our club registration day for all athletes accepting offers will be on Sunday September 20th at Frenchman’s Bay Public School in Pickering. We will be providing a club overview presentation, answering questions, collecting forms and cheques and sizing athletes for uniforms.
We will be posting complete registration details and forms here, by Friday, so please check back. Here is what we know so far, so that you can get started.

- 10:00 – 12:00 – Girls and Boys 15U/16U
- 12:00 – 2:30 – Girls and Boys 12U, 13U and 14U
- 3:00 – 5:00 – Girls and Boys 17U/18U
Each athlete must attend with a parent, and each team must have at least one coach present, preferably the head coach. The forms to be completed and Fee Schedule outlining amounts to be paid are provided below.
To prepare for registration you need to do three things:
1. Complete the necessary forms
Please print and complete these forms and bring them with you.
a) OVA Registration Form
To be completed by each athlete. When you complete it, we are “Region 4”. Your coach can help you fill in your team name, at registration. Do not pay your own registration fees! These are covered by the club.
b) Madawaska Teambuilding Camp information form and waiver
Must be completed for each athlete:

Must be completed by both parents of an athlete:
d) Photocopy of the athlete’s birth certificate
This is a requirement of the OVA and must be presented to the OVA upon request, and prior to the Ontario Championships. We retain for the season then destroy the copies.
2. Make payment for the season
a) Single registration fee. Our single registration fee varies by age group, and includes our Physical Training program and our Madawaska Teambuilding weekend (program components not offered by other clubs). The $90 increase in our overall fees from last season covers increased costs from the OVA and our suppliers, a dedicated technical director, and more competitive compensation for our coaches.
We realize that some families may need to make alternate arrangements for payment. Please discuss with Clare Masson or Larry Skelly at registration day. Our Durham Attack Cares program can help support a limited number of athletes who also qualify for external funding programs. If you wish to discuss either of these options we ask that you submit cheques as per usual but contact our athlete assistance team (AthleteAssistance@durhamattack.ca) as soon as possible to initiate a discussion.
Please print the athlete’s name and team clearly on all cheques.
We will be sizing athletes for uniforms at registration. If you are a returning athlete then determine what you need to replace in advance. Uniform costs will be provided on the fee schedule above.
