Our club registration day for all athletes accepting offers will be on Sunday September 18th at Frenchman’s Bay Public School in Pickering. We will be providing a club overview presentation, answering questions, collecting forms and cheques and sizing athletes for uniforms.
We will be posting final registration details here, by Friday, so please check back. Here is what we know so far, so that you can get started.

- 9:00 – 11:30 – Girls and Boys 12U/13U
- 12:00 – 2:30 – Girls and Boys 14U/15U
- 3:00 – 5:-00 – Girls and Boys 16U/17U/18U
Each athlete must attend with a parent, and each team must have at least one coach present, preferably the head coach. The forms to be completed and Fee Schedule outlining amounts to be paid are provided below.
To prepare for registration you need to do three things:
1. Complete the necessary forms
Please print and complete these forms and bring them with you.
a) OVA Registration Form
To be completed by each athlete. When you complete it, we are “Region 4”. Your coach can help you fill in your team name, at registration. Do not pay your own registration fees directly with the OVA! These are covered by the club.
b) Madawaska Teambuilding Camp information form and waiver
Must be completed for each athlete:

Must be completed by both parents of an athlete:
d) Photocopy of the athlete’s birth certificate
This is a requirement of the OVA and must be presented to the OVA upon request, and prior to the Ontario Championships. We retain for the season then destroy the copies.
2. Make payment for the season
a) Single registration fee. Our single registration fee varies by age group, and includes our Physical Training program and our Madawaska Teambuilding weekend (program components not offered by other clubs).
Here is a draft 2016-2017-fee-schedule We don’t expect it to change much if any, however we are finalizing costs as we finalize the number of teams and athletes for this season.
We realize that some families may need to make alternate arrangements for payment. Please discuss with a Club executive (Michael May or Barbara Bourgeois) at registration day. Our Durham Attack Cares program can help support a limited number of athletes who also apply to and are approved by external funding programs. If you wish to discuss either of these options we ask that you submit cheques as per usual but contact our DA Cares program team (DACares@durhamattack.ca) as soon as possible to initiate a discussion.
Please print the athlete’s name and team clearly on all cheques. Fill in the amounts at registration.
We will be sizing athletes for uniforms at registration. If you are a returning athlete then determine what you need to replace in advance. Uniform costs will be provided on the fee schedule above.
